Job Description:
Essential Duties:

1. Prepare medications under the direction of a pharmacist or physician.
a. Measure, mix, count out, label and record amounts and dosages of medications.
b. Prepare IV medications as per clinical trial protocol and maintain blinding parameters as necessary.

2. Review for adequate stock of instruments, supplies, and pharmaceuticals and inform the Regional Operations Manager as required to maintain adequate stock.
a. Ensure that supplies are ordered when quantities reach reorder point.
b .Maintain drug accountability logs for all medications including oral, injectable medication, etc.
c. Maintain proper storage and security conditions for all drugs as outlined and required by protocol.

3. Establish and maintain patient drug administration records for Sponsor, CRO, FDA or other industry representatives.
a. Ensure drug accountability logs are accurate and up to date by the end of the business day.
b. Follow protocol-established procedures for each clinical trial such as labeling vials and/or bottles with appropriate subject information.

4. Establish good working relationships with Sponsors, CROs and other industry representatives.
a. Ensure availability of documents, records, used and unused drug for all monitoring visits whether blinded or unblinded.
b. Be available to answer questions and inquiries from Sponsor, Monitor or other industry representative.

5. Ensure accurate completion of Case Report Forms (CRFs) for clinical trials as they relate to the Pharmacy.
a. Research and accurately complete any Data Clarification Forms that are received for the Pharmacy.

6. Follow procedures outlined by the Sponsor, CRO or other industry representative for drug destruction for the close-out of the clinical trial.
a. Maintain records of drug destruction or return shipment of drug as required by Sponsor or CRO.

Minimum Position Requirements:

Appropriate education and/or experience may be substituted on equivalent basis

Required: High School Diploma
Preferred: Certification or Technical Diploma
Field of Study: Pharmacy
Experience: 1 year hospital experience

Certifications/Licenses: Pharmacy Technician Certification
